There’s no single official answer. Mafia is a folk game with no rulebook everyone follows, so whether the Doctor can protect themselves is a house rule. The most common versions are “never,” “always,” and “once per game.” For most groups, allowing one self-save per game works best: the Doctor isn’t a free kill, but they can’t hide behind their own protection all game either.
Whatever you choose, the moderator should announce it before night one. Here’s how each version changes the game.
The four common Doctor rules #

You can combine the last two: one self-save per game, and never the same player on consecutive nights.
What changes when the Doctor can save themselves #
If the Doctor can always self-save #
The Doctor becomes hard to kill. A smart Doctor who suspects they’re a target just protects themselves every night, and the Mafia waste kills on them. That makes the Doctor almost as strong as a Detective, and the town wins more often. It also makes the Doctor’s life a bit boring, since the obvious move is to save themselves.

If the Doctor can never self-save #
The Doctor has to guess who the Mafia will target and trust someone else to stay alive. If the Mafia work out who the Doctor is, the Doctor is an easy kill. The role becomes more about reading the Mafia’s likely targets, and the Doctor has to keep their identity quiet.
This suits groups with 12 or more players, where the town has other power roles to lean on.
If the Doctor gets one self-save #
The Doctor can survive one dangerous night, usually the night after they’re forced to reveal their role. After that they’re on their own. It keeps the Doctor alive long enough to matter without making them invulnerable. It’s our recommended default.
How the Doctor’s save works with other roles #
- Bodyguard. The Bodyguard guards someone else and dies in their place if that person is attacked. If you let the Bodyguard guard themselves, the usual ruling is that it doesn’t help: an attacked Bodyguard still dies. Otherwise the Bodyguard is just a second Doctor.
- Two killers. If the Mafia and a Serial Killer both target the Doctor’s protected player, most groups rule that one Doctor protection covers both attacks. Decide in advance.
- Witch. The Witch’s healing potion saves the night’s victim once per game. Whether the Witch can heal herself is its own house rule. Most groups allow it, since the potion is single-use anyway.
- Revealed roles. If roles are revealed on death, the Mafia learn when they’ve killed the Doctor. If roles stay hidden, the Mafia can’t be sure the Doctor is gone, and they may keep wasting kills on protected players.

Should the moderator announce Doctor saves? #
That’s a separate house rule, and it matters as much as the self-save rule.
- Announce saves (“Priya was attacked, but survived”). The town learns the Doctor is alive and good at guessing. The Mafia learn their target was protected. The attacked player becomes a little more trusted.
- Hide saves (“It was a quiet night”). The town doesn’t know if the Mafia skipped a kill or the Doctor saved someone. It keeps more secrets and suits experienced groups.
Many groups announce saves in their first few games, because the Doctor’s moment of glory is part of the fun.
Tips for playing the Doctor #
- Don’t reveal early. A revealed Doctor usually dies that night.
- Protect the loudest useful voice. The Mafia tend to kill whoever is leading the town’s thinking.
- Protect a claimed Detective. If a player claims Detective and the claim sounds true, they’re the Mafia’s next target.
- Save your self-save. If you get only one, use it on the night after you’ve revealed yourself or come under suspicion.
- Mix up your picks. A predictable Doctor is easy for the Mafia to plan around.

Setting the Doctor rule in an app #
Mafia Helper has a night-screen switch labelled “Doctor can protect themselves.” It’s off by default, so the Doctor’s own name doesn’t appear as a target until you turn it on. When the Mafia’s target is protected, Resolve Night reports that player as “attacked but saved,” and you decide whether to announce it. The app doesn’t enforce limits like “once per game” or “no repeat targets,” so track those with the game log, which lists every night action in order. Being able to look back at which player the Doctor protected last night settles most of those arguments.

Frequently asked questions #
Can the Doctor protect themselves in Mafia? #
It depends on your group’s rules. Some groups forbid it, some allow it every night, and many allow it once per game. Announce your rule before the first night so nobody argues about it mid-game.
Can the Doctor save the same person two nights in a row? #
Under classic rules, yes. Many groups add a “no consecutive targets” rule so the Doctor can’t park on one player all game, which keeps the Mafia’s kills meaningful. It’s especially useful if the Detective has been revealed.
What happens if the Doctor and the Mafia pick the same person? #
That player survives the night. Depending on house rules, the moderator either announces the save (“was attacked but survived”) or just says nobody died.
Should the Doctor reveal their role? #
Usually not, since a revealed Doctor becomes the Mafia’s next target. Reveal only when it will save your life in a vote, or late in the game when proving a save will clear another player. If your group allows one self-save, keep it for the night after you reveal.
